as far as i can tell it doesn't matter. i kicked him off but my g/f pulled him up and the only real difference is the short cut scene after your decision. either he goes splat or he thanks you.

if ivan dies it doesnt affect the 100% needed to complete the game only because they give you a choice to kill him or keep him alive.

I love this mission, I always let Ivan live though. Because he wasn't such a bad guy, he get's engaged to be married and so on. But second time around I just kill him for the hell of it. The only thing different is that you won't able to do missions for him. And I don't like do any favors for Vlad, he's a prick and i'm glad he dies in the game.
