The chair was part of the Phoenix Model Pack. It's filled with lots of useful frames, sheets, and primatives of various materials, as well as a few other goodies like the chair. You can find it on the Gmod forums.
I'll give you a hint in case you try to make your own: Notice the feet. They're extremely heavy. That's the only reason why it doesn't fall over much.
