Stupid! Not a realistic scene. The guy whose tongue was cut out lies flat on his back. His wound is still bleeding which can be observed when he gets up. But if he lies flat on his back with a bleeding wound in his mouth he will suffocate, especially when he is unconscious - because the blood will fill his lungs. Blood is an essential part of such a movie but here we see that it can even spoil the logic.

And no he isn't bleeding when he is lying down, that blood is alot darker and doesn't even show the smallest bit of shining when in the subtle light, (normal blood does) so it's just dried, probably from when John sealed the guys lips.
