Linda C. McCrae had my friend sign A BLANK FORM with no wording on it. When my friend questioned this request, he was told by Linda C. McCrae that all research employees were required to sign this--now ominous--blank form. Later, my friend was even presented one of these "BLANK FORMS from Kathlyn Shumacher, the Nurse Practitioner for the CTRC, to sign. And again, the same lame excuse was presented.
