I've heard that there is a new device that would prevent these types of collisions called the Eliminator. We found out about it by google searching for the phrase "collision control communications." Has anyone else heard of this device? If so, please post info. Thanks!
ouch - 2nd leading cause of death for firefighters; thank goodness no one was killed. Word to the wise - if the A/O's nickname is "Lucky", "Crash", "Code Red Fred", etc. do not get in the truck with that person

looks like a lack of communication between vehicles and a lack of intersection skills/training, driver error/training.
