Um, it's a birth. Anybody in the room at a hospital would see everything anyway. Plus, it's TLC. If you chose to watch it, you should have realized you were going to be watching a birth. For a birth on TLC, it was very modest. And, there were no more people in the room than a usual birth to be seeing it happen. Mother in labor, husband, doctor (doula in this case), Mother-in-law and sister-in-law. At a hospital she would have had them plus at least another nurse.

If you notice they are holding her legs in the position the stirrups would keep them in. I have a 7 year old and a 4 year old and in both births I opted not to use the stirrups. Instead I held my thighs to keep my legs in a decent position because I felt it was easier for me. And coaching in many births I have seen women's feet "in the air" rather than using the stirrups. But, honestly, it is no different, other than them not being under the feet. Legs are in the same position.

I believe in a low risk moms should deliver at home with the help of a midwife. The transition from womb to outside world is much more calming for baby.
