I really liked the 'vacuforming' part. How did you do it in detail?

Heating the transparent part with the candle and then pressing the stick with the PS-sheet on it against that?

The way I did it. I stuck the template(or positive mold) on a stick, then I made the stick stand up right with blue tack, and then I heat the sheet an inch above the flame, held the sheet around each corner, when the sheet started to melt, quickly press it on the mold, use fingers to press along the edges to make it wrap around the mold.
