At 0:22 you can see its followed the derailer and not the exit line. The derailer does its job and whoevers job it was to open them looks a bit of an idiot.
it was because there were to expansion joints and the metal was internally fractured due to contraction, one of the rails sheared , can't trust network rail, oh well at least it wan't a motorail or passenger train

If this is the exit of a yard onto a main line there would be a set of trap points/ catch points.
It is like a regular set of points, but with nothing attached they de-rail anything rolling towards the main line which had not been cleared to do so.
You can see one of the rails of the trap points by the guy with the open high-vis coat.
The consequences might otherwise have been a blocked main line or far worse....
