There is a similar joke about a student that comes back to the classroom and announces that he saw an ant. The teacher realized that he taught his students how to say the word ant in english recently, so he asked the student 蚂蚁怎么说？How do you say ant? The student thought about it and replied, 蚂蚁什么都没说！The ant didn't say anything! (because he thought his teacher asked, "What did the ant say?") It's the same play on 怎么说 because it can be interpreted two different ways.

the student asks the teacher, 医生怎么说啊？(yi-sheng zen-me shuo a?) He was asking the teacher what the doctor said, but the same sentence can be interpreted as How do you say doctor?(in English) 英文怎么说 is how do you say in english. 中文怎么说 is how do you say in chinese.
