we just got a fatalgram recently at work about a miner cutting into a drill bit that was similar to this in which the gases inside the bit expanded and blew the bit up. He was not so lucky, I do hope this miner is still with us

Unfortunately the right way to do this would have been to weld the piece in place and cut it down the center to release the pressure. Then cut the 2 pieces out that you welded in.

The object that is thrown up in the air and is on fire during the blast is his welding helmet. Then you see his head slam violently against the side of the tank...then he falls to the ground. There is no possible way this poor man survived such a severe blunt trama to the head. It must have shattered his skull. It is amazing to think it can happen that fast...death. What your watching is this man's last moments on earth. Sucks.

As they lanced it, the metal tooth started to expand from the heat, but it had no where to expand to. When it let go, it was like a cork popping, all that pressure around and underneath it caused it to take the path of least resistance and it shot up and got him a glancing blow. He was out for a few minutes from what I understand.
@minetalk : when slicing tramp metal (mannie is likely correct, tooth and/or adapter trapped under pressure in the crusher) the amount of potential energy pent up by the process is enormous.
In this case the metal removed by using the magnesium lance allowed the remaining metal, heat compromised, to let fly.
I've done this many times, on various crushers and they're all nerve wracking due to the unfortunately frequent event displayed here. There has to be a better way.
Much respect to the man who dived in after the incident. Quick action saves lives.
