First of all we must understand that they are all actors pretending to be amazed. The trick can only be done with long sleeve. When he kneels down, the camera focuses away and Blaine has the time to twist his hand inwards and place it on the ground. Then all he has to do is to slowly twist it back.

It's all in the position of the hand! eventhough there positon looks the same, Davids hand actually has gone a half cirkel when he starts, so he's at the beginning again while the doctor can't go no further and David has a half cirkel left cuz he's at the start of the cirkel just that their hands look the same but david's arms is in another position! Easier way to say it is: stop at 0:24 and you can clearly see that the doctors elbow are facing outwards while davids are facing inwards! =) Easy.

they also do the same trick with a dove, but basically, you push the head of the bird down underneath its wing, and you have a fake head in your palm, hiding it. And when people are freaking out about the head disappearing you make the fake head visable and wave it around.
