I figure if you have headphones on (Big ones in my case...i love bigass headphones), you can be affected by the Infrasound (?) that the movie projects...other than that, out loud, i dont think it would penetrate...well then again we can hear VERY subtle sounds as well. But all in all, the infrasound works much better in headphones of course. When i seen the first brutal scene (to not spoil), i was in tears right after. NO JOKE! I had my headphones on...go figure.
