I think he tripped. Look at his left foot at take-off; it looks like he hit the ground with the very tip of his foot, almost like he stubbed his toe. That would explain why he didn't jump and why he took the extra step: He messed up on his last step, and it made him trip forward. I don't know if that's what actually happened, but it looks like it to me
