Vincent was having a glitchy seizure, hence the reason why this happened. he walked off screen, tripped and drowned which is why the scene was ruined.

Has nobody seen Final Fantasy: Mixed in Balamb II? It explains everything. Basically, whenever Sephiroth successfully kills someone or something with his sword, the sword prevents anyone from coming back to life. After all, when you saw the blade go through Aeris, you didn't see any blood, did you (and it's not like there were problems showing blood in this game either)? Not only that, but it looked all mystical and stuff when it went through her chest.
