Roach and Ghost are not dead, because at some point on the mission "Loose Ends" you can hear Ghost say: "Why do we always have to wear these fireproof vests?" and also if you look closely you will notice the bullet that went through Ghost was by the area of the hip not the face as it may seem, and probably if they had fireproof vests and they were both hit in the area of the hip they probably survived
