Similar thing happened to me, I was on sandbox and I was the only person in the game. I placed a warthog down went to get in it and it drove off and splattered me but it just went in a straight line and got destroyed by the lazers

Theres a glitch where if throw a nade and switch to monitor mode halfway through the animation, after a few mins, it will just appear where it should of been thrown, so he did this and just waited for the nade to appear
