It's called Burning Heat and it's on DDR. Dunno which DDR, one of them at least.

It's a remix of the Burning Heat theme from Gradius Two.

@roflburger1 It's the Renamon 2007 model, I just reskined it to my own personal playermodel using VTFEdit and Gimp 2.6, and then hexed it so that I can have both my model's addon and the Renamon 2007 model addon at the same time.
