In my opinion the Foley Chokeslam through the roof was definitely planned, its no coincidence that the camera was in position, the cell was rigged, even that the Undertaker chokeslammed him forward as opposed to straight down. I've heard testamonies from workers/wrestlers before that Foley lied before to protect his wife, who was fuming at the first big fall and was pretty much going to take the kids and leave him if he had agreed to a second big fall.

@Jberm0o0 If I remember correctly from his biography, the spot at 1:00 was plan, but the one at 1:40 was supposed to be a chockslam on the top of the cage on the chair.... i'm not 100%
@qwertyguy i thought that mick foley dislocated his shoulder and when he got chokeslamed the chair hit his lip and the reason he was smiling was because he was sticking his tougne through the hole in his lip
