i think derek is sad because he feels like he is never good enough for meredith and he has realized that he does in fact fail her. i love it because its like his epiphany or something. why she doesn't trust him and all that. i love them in this episode!!!! They are amazing.

well initially you think its because he is losing all of the patients and it is but its also about the fact that he's not only failing himself but he's failing meredith. also in this episode dereks very anti ]optimistic so thats another reason he's so upset.
