Despite of the low cost, the quality of care here is much higher.

It's a very malleable hybrid, which has different implications when it moves in either direction. In this case, we're definitely more of a republic. Big government by representatives equals less say from you and me. Try to bring it down and it'll be the Whiskey Rebellion all over again.
