a photocopy of this sheet was sent to me from my staffing firm , so I don't know precisely, but according to the caption under the sheet, this is published by YAMAHA, and I think the name of the score book is [ディズニー マジカル・メロディー ～アラン・メンケン・ベスト～].

And...sorry, I arranged the score quite a little.
