He turned the pen cap towards the back of his hand and is just using the rubbing motion as as distraction. The top of the pen (pen cap) is hanging on the top part of his hand.

just figured it out, easy. fold the top part of your hand (when fingers pointing up) in towards ur palm very slightly, just to create some skin/fat or w.e in that part of ur hand.

hold ur hand out on a slight angle (when hand is facing audience, very hard to see) and the rubbing of the pencil on that part of hand allows it to 'dig' into your hand a bit, and will rest on the fat-part of your hand allowing it to stay there until you make a slight move.

@DopeBass You gut the fat part correct, but try pushing the pencil very hard against the hand while rubbing vigorously. That way it is more believable because you don't have to tilt your hand at all. The sweat and static/friction will keep it on.
