You can get a US passport, using the HI COLB. A passport only identifies the bearer as a US citizen, subject to the US Constitution, and treaties there under. Neither the HI COLB,

nor a passport based on it, can be used to differentiate between a native citizen, a natural

born citizen, or a naturalized citizen. Only on examination of the vital statistic record(s),

and/or a certified copy of the vital statistic record(s), can a determination of the category of citizenship be ascertained.

This is another fake IMO. If you'll notice the signature of the doctor, the last name starts with a "C". Also, the signature under that has only one name. Nobody should sign an official document with one name.
